Online Forms
Thank you for selecting Prime Cremation. For your convenience you may now complete all the Final Arrangements online and via fax. Before you begin you must contact Prime Cremation 250-545-7944 and we will issue you a file number required to begin the arrangement process. Once we have received the forms one of our Licensed Funeral Directors will contact you by phone or at the start of the next business day. At any time you are welcome to contact us via telephone.
Instructions
The first document you should read is the INSTRUCTIONS. You can print or save the PDF document and refer to it as you complete the forms on your computer. After calling Prime Cremation, you can proceed to download the forms below.
After reviewing the instructions complete all 5 forms, then sign and or initial where required.
*Important* The following forms must be completed by the person who has the Right of Disposition. You must include a photocopy of photo identification with your forms. Please feel free to call us if you have questions or require clarification.
Form 1:
Notification of Death. This form provides the basic information to start the arrangements process.

Vital Statistics Registration of Death. This is required to register the death with the provincial government.
Form 4:
Authorization for Cremation. This form authorizes the deceased to be cremated at Pacific Crematorium.
Form 5:
Goods and Services Contract. Here you will select the services you require, how many documents you need and what type of urn you would like.
